Dear Campus Community,
All students, faculty, and staff must be fully vaccinated to enter campus. Fully vaccinated means two weeks have passed after your last dose (2 weeks after Johnson & Johnson or 2 weeks after 2nd dose of Pzifer or Moderna).
Students must submit vaccine cards to https://www.studentehr.com/.
Faculty & Staff must submit vaccine cards via the Health Coach Daily Symptom Check. Instructions can be found here.

Security will also ensure that all faculty, staff, and students complete the Health Coach Daily Symptom Check each day via mobile device. To sign up, please click this form.
Again, all faculty, staff and students will need to complete this each day you enter campus.
Masks are required while on campus. Please visit security at the main gate to obtain free masks.
There will be no exceptions to these policies and you will be turned away.
To request accommodation, please complete and submit the Fall 2021 Remote Class Access Accommodation Form. For all questions, please contact Disability Services at email@example.com. Any accommodations granted for remote access to courses are for all courses and students cannot access campus facilities.
Woodbury will also have weekly testing available through the end of September. This will be re-evaluated two weeks into the semester. We highly encourage everyone to participate in the testing but will not make it mandatory as we have mandated the vaccine to access campus. However, this may change as necessary. Weekly testing is due to the fact that we will have many community members returning to campus from outside of LA County, so this will help us catch any vaccinated individuals who may be asymptomatic and test positive.
